Dr. Bonakdar Explains: Some people are very expressive and animate their facial muscles as they communicate, whereas others are not expressive and have their poker face on all the time. Contraction of the frontalis muscle or the muscles of the forehead can be an outward sign of surprise or repulsion. Botox Cosmetic administered properly in the forehead muscles, diminishes the forehead wrinkles without interfering with upward movement of the brow which is needed to apply contact lenses or eye makeup. I am often asked to get rid of the last bit of wrinkles above the tail of the brows or what I call “commas”. Dr. Bonakdar Explains: This is a 47-year-old female with overactive muscles between her brows and on her forehead who is treated with Botox Cosmetic. Overactive muscles fold the overlying skin with each contraction and eventually breaks down the collagen in the skin fold which leads to formation of static expression lines. Close examination of the after-photo reveals that even though the hyper expressions have been eliminated, there are several horizontal lines in the forehead and a vertical frown line between the brows. Static lines that remain after Botox treatment is due to damage of the overlying skin and need further consideration. If the skin damage has caused reduction of skin thickness as in this patient's glabellar area (between her brows), I recommend filling this line with a filler. If the skin damage is early like her forehead lines, repeat Botox treatments every 3 to 4 months will allow the skin to rebuild its missing collagen and repair this early damage in the next 12 months.
